License Renewal

All Active licensees can renew online between January 1st and March 31st. Please click here to renew online.

If you prefer to renew via postal mail, please download, print and complete the form posted on this page. Renewal forms can be mailed to 1000 Centre Green Way, Suite 200, Cary, NC 27513. If paying by credit card, you may fax your renewal forms to us at 919-882-1776.

Ethical Responsibility: It is your ethical responsibility to answer all of the legal questions on your renewal application truthfully. Any false statement found by the NCBDN will be treated as a violation under North Carolina General Statute § 90-363(a)(1). Should the NCBDN discover that you have violated N.C. General Statute § 90-363, upon a hearing, the NCBDN reserves the right to refuse renewal of your license. You MUST inform the Board of your status, even if you are NOT RENEWING your license.
Please determine which status applies to you:

  • Choose to go “Inactive.” Inactive status is for those licensees who will no longer be practicing dietetics/nutrition in NC, but who foresee the possibility that they may return to practice in NC someday. Inactive status is available only to Active licensees, before their license expires on March 31st. If you go inactive, you will not pay the yearly renewal fees, until the time you wish to reactivate, at which point you will only pay the current year’s renewal fee.

To go Inactive you must:
1. Complete the Inactive Status Request Form, found here.
2. Return your original certificate. If you no longer have your original certificate, you may instead submit the Affidavit of Lost/Stolen Property, found here.

  • Choose to “Relinquish.” Relinquishing your license means you will no longer be practicing dietetics/nutrition in NC, and have no plans to return to practice in NC again. If you decided to practice in NC again, you will be required to reapply, and pay the full issuance and application fees. Relinquished status is available to all licensees at any time.

To Relinquish you must:
1. Complete the Relinquish Form, found here.
2. Return your original certificate. If you no longer have your original certificate, you may instead submit the Affidavit of Lost/Stolen Property, found here.
