If you have lost/misplaced your card or certificate, please complete the form posted here and mail or fax with your payment ($30 for certificates, $20 for ID cards). With all requests for replacement certificates, you must also submit the Affidavit of Lost/Stolen Property as the legal replacement for your original certificate. This form requires you to obtain a notarization. Please mail all documentation to NCBDN, 1000 Centre Green Way, Suite 200, Cary, NC 27513.
