If you will no longer be working as a licensed Dietitian/Nutritionist in North Carolina, we ask that you complete and mail the Relinquish Form below and enclose your original certificate to Relinquish your license. If you no longer have your origrinal certificate, please submit the Affidavit of Lost/Stolen Property instead.
If you may return to North Carolina to practice Dietetics/Nutrition in the future, please read about Inactive Status.
Once a license is relinquished, to return to practice in North Carolina, you will need to reapply and will be subject to the full application and issuance fees.
If you do not inform the Board of your status, and you do not renew your license, your license will be considered Lapsed, and you will be subject to our Lapsed License Procedure.
