After March 31st and until May 31st, you may renew by mail. The $75 late fee now applies to all renewals before May 31st, for a total renewal cost of $150. A renewal form is posted below as a PDF. Please print, complete and mail it to 1000 Centre Green Way, Suite 200, Cary, NC 27513. If you are paying by credit card, you may fax the form to 919-882-1776.

If you are Inactive and wish to Reactivate, please contact the Board office at .(JavaScript must be enabled to view this email address) or 919-228-6391 for further instructions.
If you will no longer be practicing dietetics/nutrition in North Carolina, you must inform the Board of your status.
You have two options:
Relinquish your license
If you believe you will not return to practice dietetic/nutrition in NC again, then you may want to Relinquish your license. Should you want to practice dietetics/nutrition in NC again, you will have to reapply for the full fee of $175.
To Relinquish your license please follow the steps below:
1. Complete the Relinquish Form and
2. Return your original certificate (not the plastic ID card). If you no longer have your original certificate, you may instead submit the Affidavit of Lost/Stolen Property.
Go “Inactive”
Inactive Status is now only applicable to those who are currently Active with an expiration dates of March 31, 2014. Inactive status puts your license on “hold” (you do not pay the yearly renewal fee) until the time you wish to reactivate. At which time, you only pay the current year renewal of $75. You may only go Inactive while your license is active, before the March 31st expiration date.
To go Inactive, please follow the steps below:
1. Fill out & notarize the Inactive Status Request Form and
2. Return your original certificate (not the plastic ID card). If you no longer have your original certificate, you may instead submit the Affidavit of Lost/Stolen Property.
